Thomas Allen's Long-Term Care Case Management Division is Hiring!

LOCATION:    Office in West Saint Paul, MN. Travel throughout the Twin Cities is required for meetings and visits with individuals served. Based on caseload, travel throughout other parts of Minnesota may be required. Temporarily working remotely.

HOURS AVAILABLE:    Flexible; generally Monday-Friday 8am - 4:30pm

JOB SUMMARY:   As a Case Manager, you will create and review care plans, and coordinate assessments and services with individuals served and the interdisciplinary team. You will also connect individuals served with community resources and ensure waiver coordination and documents meet lead agency guidelines. This position will work with a combination of CADI, BI, and CAC waivers. Each Case Manager is an advocate for the individuals they work with and helps to make a difference in their lives.

REQUIREMENTS:

  • 4-year degree in social work and LSW, or Registered Nurse, or Public Health Nurse, or meet the minimum qualifications of a social worker under the MN Merit System with a 4-year degree in a related field such as psychology, human services, or sociology
  • Must have at least two years full-time social service experience in the education or treatment of persons from vulnerable populations.
  • Background clearance
  • Ability to effectively communicate in English, written and verbally
  • Driver's license, reliable vehicle you can use for work purposes, acceptable driving record and proof of auto insurance
  • Must be at least 18 years of age

NICE TO HAVE:

  • Experience in CADI, BI, EW, and CDCS
  • Knowledge of MA, Medicare, and MN Healthcare programs
  • Knowledge of community resources and providers
  • Great organizational skills and attention to details
  • Knowledge of computer basics and familiarity with Microsoft Office suite
  • Bi-lingual oral and written skills in Spanish, Hmong, or Somali

IN THIS POSITION YOU WILL BE COMPLETING THE FOLLOWING:

  • Visits with persons on the caseload and attending team meetings
  • Completing individual service plans (CSSP)
  • Assisting with planning for new service development
  • Locating residential, vocational, and other needed services for individuals served
  • Monitoring service delivery and ensuring the health and safety needs for individuals served
  • Completing case notes and documentation
  • Acting as advocates for individuals served
  • Required trainings, both upon hire and annually
  • Participate in creating an overall positive community atmosphere
  • Other duties as assigned
