LOCATION: Office in Plymouth, MN with frequent travel for visits with individuals served in Wright County.

This position is in-person. Hybrid work-from-home opportunities are available after the first 6 months of employment!

HOURS AVAILABLE: Monday - Friday 8am - 4:30pm

BONUS: $250 paid at hire, $250 paid at 3 months, $500 paid at 6 months, $500 paid at 9 months

STARTING WAGE: $46,654.40/year - $52,166.40/year DOE

JOB SUMMARY: As a Case Manager, you will conduct assessments, create and review care plans, and coordinate services with individuals served and the interdisciplinary team. You will also connect individuals served with community resources and ensure waiver coordination and documents meet lead agency guidelines. In this position, you will be working with CADI and DD waivers.

REQUIREMENTS:

• Bachelor's degree in social work and LSW, or a bachelor's degree in a related field such as psychology, human services, or sociology, or Registered Nurse, or Public Health Nurse
• Must have at least 1 year of full-time social service experience working with DD
• Experience with Rule 185 and DD Waiver preferred
• Knowledge of MA, Medicare, and MN Healthcare Programs strongly preferred
• Background clearance
• Ability to effectively communicate in English, written and verbally
• Driver's license, reliable vehicle you can use for work purposes, acceptable driving record, and proof of auto insurance required
• Must be at least 18 years of age
