JOB SUMMARY: The Program Coordinator works cooperatively with the Program Manager to complete support functions at assigned divisions that include assuming a leadership role serving as support team liaison, while delegating workflow and supporting co-workers in the absence of the Program Manager.

Specifically, Program Coordinator responsibilities include:

• Maintain a good working knowledge of every individual in the home, develop, review, and monitor implementation of service outcome plans
• Provide training and support to new staff
• Complete necessary paperwork, reports and assessments, review documentation and logging, provide follow up with staff
• Schedule necessary appointments and provide or arrange transportation
• Participate in and facilitate various regular meetings, act as a coordinator with vocational employers and day program personnel
• Work with the program manager on assisting with finances
• Regularly communicate with the nurse consultant and program manager and work cooperatively with them on any assigned tasks
• Direct care tasks
• Activity planning

REQUIREMENTS:

• Minimally, must meet the qualifications of a Designated Coordinator:

1. A Bachelor's (BA) degree in a field related to human services and 1 year of full-time work experience providing direct care services to individuals with disabilities or those who are age 65 or older; OR
2. An Associate's (AA) degree in a field related to human services and 2 years of full-time work experience providing direct care services to individuals with disabilities or those who are age 65 or older; OR
3. A diploma in a field related to human services from an accredited post-secondary institution and 3 years of full-time work experience providing direct care services to individuals with disabilities or those who are age 65 or older; OR
4. A minimum of 50 hours of education and training related to human services and disabilities and 4 years of full-time work experience providing direct care services to individuals with disabilities or those who are age 65 or older under the supervision of a staff person who meets the qualifications identified in 1 to 3 above.

• The individuals served in the home prefer female staff
• Must demonstrate competency in the completion of the above noted job responsibilities
• Individual must be mature, polite, competent, and possess team playing abilities, effective listening skills, and strong overall social and communication skills
• Current First Aid and CPR Certificate as required by the home/division
• Department of Human Services background clearance
• Valid driver's license, reliable vehicle, acceptable driving record, and proof of auto insurance required
• Ability to effectively communicate in English, both written and verbally
• Must be at least 21 years old
