LOCATION: Office in West St. Paul, MN with frequent travel for visits with individuals served in Dakota County.

This position is in-person. Hybrid work-from-home opportunities are available after the first 6 months of employment!

HOURS AVAILABLE: Flexible 8am - 4:30pm, Monday - Friday

STARTING WAGE: $45,177/year - $50,523/year DOE

JOB SUMMARY: As a Case Manager, you will conduct assessments, create and review care plans, and coordinate services with individuals served and the interdisciplinary team. You will also connect individuals served with community resources and ensure waiver coordination and documents meet lead agency guidelines.

REQUIREMENTS/MUST HAVE:

• Bachelor's degree in social work and LSW, or a bachelor's degree in a related field such as psychology, human services, or sociology, or Registered Nurse, or Public Health Nurse
• Background clearance
• Ability to effectively communicate in English, written and verbally
• Driver's license, reliable vehicle you can use for work purposes, acceptable driving record and proof of auto insurance
• Must be at least 18 years of age

NICE TO HAVE:

• Experience in CADI, BI, EW, CDCS and AC
• Knowledge of MA, Medicare, and MN Healthcare programs
• Knowledge of community resources and providers
• Great organizational skills and attention to detail
• Knowledge of computer basics and familiarity with Microsoft Office suite
• Bi-lingual oral and written skills in Spanish, Hmong, Somali
