Hiring a Program Director!

JOB SUMMARY: As the LTC Program Director, you will be responsible for the oversight and overall operation of the Long Term Care Case Management division.

Responsibilities Include:

• Recruitment, orientation, ongoing training, supervision, and evaluation for persons supervised
• Review and approve requests for training, sign off on paid time off hours and employee performance reviews
• Maintain agreements with agencies for the provision of interpreters (language and deaf and hard of hearing).
• Assist with auditing files for County and DHS audits
• Ensure policy and practice are in compliance, work with internal departments to create policy as needed
• Ensure completion of required reports
• Work with budgets and spend-downs
• Perform other duties as assigned

REQUIREMENTS:

• Bachelor's degree in social work and LSW, or a bachelor's in a related field such as psychology, human services, or sociology, or Registered Nurse, or Public Health Nurse
• A minimum of 2 years of experience in human services is required
• Knowledge of Minnesota Health Care Programs, CADI, BI, EW, and the AC Program is required
• Willing to drive own vehicle to perform job duties
• Background clearance
• Ability to effectively communicate in English, written and verbally
• Driver's license, acceptable driving record, and proof of auto insurance required
