LOCATION: Burnsville, MN

HOURS AVAILABLE:

JOB SUMMARY: In this position, you will work cooperatively with the program manager to complete support functions at assigned divisions. This includes assuming a leadership role serving as support team liaison while delegating workflow and supporting co-workers in the absence of the program manager.

DUTIES INCLUDE:

  • Completing assessments
  • Scheduling and attending various meetings
  • Coordinating and/or completing all paperwork relative to service reviews
  • Developing and revising personal records, health care plans, etc. regarding the individuals served
  • Serving as primary liaison with vocational employment and day program personnel
  • Proving medication administration training and assisting with the monitoring of administration and documentation, and ensuring medications are stocked
  • Helping to train and perform orientation with new hires
  • Scheduling appointments, filling out medical forms as needed
  • Helping with financials
  • Activity planning
  • Direct care
  • Other duties as assigned

REQUIREMENTS:

  • An associate degree in a field related to human services or public relations
    • OR 2 years of full-time work experience providing direct care services to individuals with disabilities or age 65 or order
    • OR a diploma in a field related to human services from a postsecondary institution AND 3 years of full-time work experience providing direct care services to individuals with disabilities or age 65 or older
  • A minimum of 50 hours of education and training related to human services and disabilities and 4 years of full-time work experience providing direct care services to individuals with disabilities or age 65 or older under the supervision who meets the qualifications identified above
  • Ability to utilize computer programs and applications such as word processing and email
  • Exceptional ability to work effectively under pressure and meet deadlines
  • High levels of integrity, competency, and sense of urgency
  • Flexibility and nimbleness in changing environments and situations
  • Superior organizational skills
  • Department of Human Services background clearance
  • Valid driver's license, reliable vehicle, acceptable driving record, and proof of auto insurance required
  • Must be 18 years or older
  • Ability to effectively communicate in English, both written and verbally
