This position is in-person. Hybrid work-from-home opportunities are available after the first 6 months of employment!

HOURS AVAILABLE: Flexible Monday - Friday 8am - 4:30pm

JOB SUMMARY: The Senior Case Manager has the primary responsibility of assisting their peers with onboarding, ongoing case consultation, and shadowing as well as providing Case Management services to a variety of populations, following applicable County, State and Federal guidelines.

Senior Case Management specific tasks

1. Assist with the orientation and ongoing training of all assigned professional personnel, including but not limited to providing shadowing opportunities of meetings and follow up paper work.

2. Assist management to ensure each assigned employee is provided orientation to all items on the orientation document form.

3. Complete peer internal quality audits with assigned employee during orientation period and as needed. Ensure case managers are provided additional training and understand the audit requirements.

REQUIREMENTS/MUST HAVE:

  • Individual must be meeting all job expectations.
  • Must be at least 18 years old.
  • Must demonstrate competency in the completion of the above noted job responsibilities.
  • Background Clearance for licensing agencies.
  • Must be able to drive own vehicle to perform job duties.
  • Valid Driver's License. Must have and maintain a clean driving record. Refer to "Safe Transportation" policy.
  • Submit current automobile insurance upon hire and prior to each renewal.
  • Individual must be mature, polite, competent, and possess team playing abilities, effective listening skills and strong overall social and communication skills.

Must meet one of the following three requirements:


•Licensed as a Public Health Nurse under Minnesota Rule 6316.0100 and Minnesota Statute, section 148.171, subdivision 18; OR
•Licensed as a Registered Nurse under Minnesota Statutes, Chapter 148, sections 148.171 – 148.285; OR
•Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field OR a graduate from an accredited four-year college with a major in any field and one year experience as a social worker in a public or private social service agency; AND maintain licensure by the Minnesota Board of Social Work once license is obtained. Meeting the minimum qualifications of a social worker under the Minnesota Merit System (per Minnesota Rule 9575) may substitute for licensure by the MN Board of Social Work.

NICE TO HAVE:

•Have at least one year of HCBS case management experience
•Has demonstrated willingness to take on additional responsibilities such as, MNSP mentor, RMS mentor, employment mentor or hosting an intern.

To Serve Persons with Rule 185 Case Management

  • Must have at least 1 year full time equivalent of experience in the education or treatment of people developmental disability or related conditions diagnosis.

To Serve Persons on a CAC Waiver:

Must meet one of the following three requirements:

  • Licensed as a Public Health Nurse under Minnesota Rule 6316.0100 and Minnesota Statute, section 148.171, subdivision 18; OR
  • Licensed as a Registered Nurse under Minnesota Statutes, Chapter 148, sections 148.171 – 148.285; OR
  • Graduate of a School of Social Work AND maintain licensure by the Minnesota Board of Social Work once license is obtained. Meeting the minimum qualifications of a social worker under the Minnesota Merit System (per Minnesota Rule 9575) may substitute for licensure by the MN Board of Social work.
